- Parts 1 and 2 (included in link above) are Required for all Participants.
- Part 3 is only required for participants attending events that last longer than 3 days or activities that are deemed more strenuous.
- If an event you plan on attending requires a Part 3 medical from, download it here.
- If you have grounds to request exemption from the required immunizations, please complete the Immunization Exemption Request.
- All participants are required to complete, acknowledging their understanding of Institute policies and procedures.
- New recruits must fill out a measurement chart to ensure that they receive uniforms that will fit properly.
